Understanding Your Anthem Medical Plan

Who do I contact for the following?
How do I find a provider (Primary Care or Specialist doctor, Hospital)?
Follow these steps:
- Go to – https://www.anthem.com/ca/find-care
- If you are registered as a member, you will be prompted to login and complete your search
- Non-registered members may select Continue under Search as a Guest
- From the What type of care are you searching for? drop down menu, select Medical
- From the What state do you want to search in? drop down menu, select your desired state
- From the What type of plan do you want to search with? Drop down menu, select Medical (Employer-Sponsored)
- From the Select a plan/network drop down menu, select Blue Cross PPO (Prudent Buyer) OR Select PPO – Small Group as your network
- Enter search parameters (provider type, zip code, name, etc.)You may also call (855) 383-7248 for an Anthem representative to assist you.
Am I required to select a Primary Care Physician (PCP) on the PPO Plan?
No, you do not need to select a PCP on the PPO plan because you do not need PCP referrals to go see a specialist.
Do I need a referral to see a specialist?
No, you can see any specialist at any time. If you wish to go see a specialist who is an out-of-network doctor, you may incur higher out-of-network costs.
If I need to fill a prescription, what pharmacy do I go to?
Follow these steps:

Can I change my plan or drop coverage?
Once you enroll into a plan, you cannot change or drop unless you experience a Qualifying Life Event. Some examples of a Qualifying Life Event are: birth of a child, marriage, or loss of coverage.
After selecting my plan, how soon can I receive my ID card?
Once enrolled and Anthem processes your enrollment form, you should receive your card within 7 to 10 business days.
I have a question regarding my Anthem claim, who can I contact?
Can I access my medical benefit information online?

For Members
- Medicare, Individual & Family, and Employee group plans
- Click on Register Now
- Enter your information
- Select Identification Type: HCID/Member ID – on your ID card
- Enter HCID/Member ID
- First Name
- Last Name
- Date of birth
- Click on Next
Are there any medications that may need prior authorization?
Yes, if the medication is either “new” to the market, experimental, or has certain limits you will need prior authorization. Your doctor may need to call or complete a form for prior authorization or contact Anthem’s provider service number.
Do I need prior authorizations for MRIs or other procedures such as surgeries?
Yes, some procedures, including but not limited to surgeries, will require prior authorization. Please refer to your Benefit Summary to see which benefits may require prior authorization or contact the member service number on the back of your card at (800) 274-7767
What is the difference between Select PPO and PPO?
There are 2 PPO Networks of doctors – The Select PPO Network (smaller), and the Prudent Buyer/Full PPO Network (larger)
What is EPO?
Provider Organization – The EPO Network plan uses the EPO Network of doctors, and does not cover care at Non-Network providers.
What is a HDHP?
HDHP is a High-Deductible Health Plan, which is H.S.A. compatible.

- Select your search type – Doctors or Locations
- Enter your region – California – Northern or California – Southern or enter your zip code
- You may also enter additional search criteria such as Health Plan (HMO), Provider Type and Hospital, Doctor’s name or medical specialty
- Click Search – a list of providers in your area or the surrounding areas will appear.

- Log onto kp.org
- Choose your region at the top
- Select Doctors and Locations
- Click on choose your doctor then “Go to Search”
- If you need assistance you can call member services at (800) 464-4000 and a Kaiser representative can assist you
